چکیده

Strategies involving new applications of existing drugs are urgently needed to reduce the time required to cure patients with drug-susceptible and drug-resistant tuberculosis (TB). Use of high-dosage rifampicin is one such approach. Recent data from preclinical animal models (Hu et al., 2015) and clinical studies (Boeree et al., 2015) support a potential role for highdosage rifampicin in TB chemotherapy, although more studies are required to guide optimal clinical management. Specifically, further basic preclinical research is needed to: (i) Develop pharmacokinetic/pharmacodynamic models to improve our understanding of drug bioavailability and activity in tissues; (ii) Determine the antimicrobial efficacy and the ability of high-dosage rifampicin to reduce the emergence of antibiotic resistance; and (iii) Explore high-dosage rifampicin for evaluation of new combination regimens to achieve the ultimate goal of shortening TB treatment and achieving stable cure without relapse.

The background and rationale for a new fixed-dose combination for first-line treatment of tuberculosis in children.

In 2010, the World Health Organization revised the recommendations for the treatment of tuberculosis (TB) in children. The major revision was to increase isoniazid, rifampicin and pyrazinamide dosages according to body weight in children. The recommendations for higher dosages are based on consistent evidence from 1) pharmacokinetic studies suggesting that young children require higher dosages ...

High-dose rifampicin kills persisters, shortens treatment duration, and reduces relapse rate in vitro and in vivo

Although high-dose rifampicin holds promise for improving tuberculosis control by potentially shortening treatment duration, these effects attributed to eradication of persistent bacteria are unclear. The presence of persistent Mycobacterium tuberculosis was examined using resuscitation promoting factors (RPFs) in both in vitro hypoxia and in vivo murine tuberculosis models before and after tre...
